My fiance has been divorced since 2002 and has two children that live in Missouri with his ex. They have no custody agreement yet and their child support does not say anything about who gets to claim the exemption. What happens if they both claim the children on the return? He pays 71% of the child support. Without her signing over the exemption (she never will) can get get it?

No, he can't. He needs to go to court to have the exemption awarded to him, and they will force her to sign the form.
