What is the name of your state? I'm from Canada

Awhile back I filled out a mortgage disability insurance application for disability coverage on my mortgage and gave it to the bank that holds my mortgage for processing with their carrier. This year I became disabled and filed a claim. The claim was denied because they said they have no record of the application. I produced my copy of the insurance application and they still denied it because they say the bank has no record of it. It seems to me that the bank lost my original application form - however, how can they deny my claim if I produced my copy.
