What is the name of your state? CA
I am currently on unemployment (2mon)
I am going to rehab and want to know if I am eligible for
state disability.

Assuming California has some sort of State disability plan (I'm not in CA and I don't know), what I can tell you is that you cannot collect UC benefits and disability benefits at the same time. That would be fraud.

To be eligible for UC benefits, you must be available and physically capable of working. To be eligible for some sort of disability benefit, a doctor must certify you as being medically unable to work. Obviously, you can't be both.

You are only eligible for disability if the doctor has said you cannot work and if you have made at least $300.00 in the 12 month period they will go by.
