[What is the name of your state? Pennsylvania---Ajudgement was entered in our favor concerning monies owed to use by a previous tenat at our rental home. The tenant has filed an appeal. On the notice sent to us by the previous tenant it states "Praecipe to enter rule to file complaint and rule to file". Stating that we must file a complant within 20 days. What is our next step? The amount owed is small $375approximately.
Do we need to contact a lawyer or can this be handled on our own?

Your 'next step' is to file an answer to the appeal notice with the court within 20 days of receipt. Then ask for a new court date where you get to start all over again.

I know this sounds really stupid but---Does this, complaint/ appeal have to be on a certain form or letterhead and what are we to state in this complaint.

If it remains in the small claims (informal) court, which is unlikely, no. However, if your case has been appealed into a higher (formal) court, then yes. It should be in the legally correct form/style.
However, in this case... any answer is better than no answer.... as you really just want to get this before the court. So, send the court and the appellant a very simple letter denying their claim.... and asking that a hearing be set as soon as possible.

and what are we to state in this complaint.

We filed the appropriate conplaint letter, the prothonotaries office would not tell us if it was necessary to send a copy of the compaint to the appellant. Do we need to send a certified letter or will the courts tqake care of it?

Send certified RRR and a copy by first class (include reference in letter).

Example:
"Sent USPS Certified: Receipt # _______________________________
Also sent by USPS first class mail"
