I am in Oregon and need to file against someone who owes me for a NSF check. I have run it thru the bank twice. I have tried to collect from the person by sending certified letters. As I fill out the form I find there is no place to add the returned check charges or interest (as per the regestered letters of 1.8% per month stated). How do I go about requesting that amount?

Actually, rather than respond to an illegal (violation of forum TOS) solicitation on the forum, why not let your local DA handle it for free??
Each county DA has their own 'bad check enforcement' division.... Just contact your county DA's office and file a complaint.
