I am disabled with a back injury sustained at work on 8-23-05. LTD was awarded after weeks of the Insurance Co. going back to my doctor for additional information, but now they have sent me additional forms to complete. I am concerned about my right to privacy being voilated by some of the information they are requesting, ie. my marital status, husband's name, DOB, & SS#, names of doctors I have seen over the past two years, regardless of reason, etc.
My Workers' Comp attorney advised me NOT to sign the broad Authorization to Disclose Information About Me form that came with that packet and now it is back again.
In a 6-page Personal Profile, they are also asking me for educational and work histories.
My question is, how much of this am I required to give them in order to keep my LTD benefits?
