What is the name of your state? Ohio
In 1996 was divorced and awarded half of my ex-husbands retirement($10,000). PERS would not acknowledge it....But my ex passed away last september. Does anyone know if there is any forms I could file on behalf of myself since I cannot afford an attorney and do not qualify for discounted legal help?

If you already have the order signed then contact the administrator of the retirement plan. They should have a copy as well, and find out what you have to do to get your part of the money.
