Hello from Massachusetts,

I provide a service to businesses (training). For some clients I provide just one training in a year, but others I do multiple. My business is setup as a sole proprietorship. (I know, not the best one.) Is there a form I can ask the businesses for similar to a W2 that would summarize (and make official) all of their payments to me over the year? (I have to pay taxes on their payments to me.)

Thanks!

Since you are providing a service, it would be normal for the companies to provide you a form 1099. However many will not if their payments to you are not greater than 600.00 for a given year.

Its your responsibility to keep good books to track your income, whether you are provided a form 1099 or not.
