What is the name of your state? New York

Hello, I have question concerning the legalities of a proxy. My mother was taken to the hospital by my nephew, where she was reduced to a very weak state, unable to write or make decisions regarding to her medical care. My nephew claims my mother in this state, was able to sign papers making him proxy, giving him full rights over her financial and medical care. I am very suspicious of my nephew's concern for my mother's health and the legalities of the proxy. I want to know if I can challenge the proxy or get a copy of the proxy. I really do not know what to do regarding the situation and I would like some help.
