What can I do to get supervised visitation for my children?

My ex physically and mentally abused me on and off for the 11 years we were married. He still continues to try and control me by financial means. He is not physically abusing my kids, but he is mentally abusing them. My daughter is seeing a psychologist due to past and present difficulties with her father. My son is acting out in school and at home to show his anger.

My ex last had them at Christmas time for a total of four days and the ramifications are long-lasting and we are still trying to deal with them.

I would like to know what I would have to do in order to get supervised visitation for my children and/or get him removed from custody of the children. He lives out of state (GA, we live in NY) and currently I have full physical custody and he has joint legal custody.

He wants to fly the children unaccompanied (they are 11 and 6) down to GA for summer vacation. I want to have something done well in advance of this upcoming event. Any advice would be greatly appreciated.
