I had a universal life insurance policy for about 7 years and "cashed" it in last February. This policy had a guaranteed death benefit and a "contract fund" that grew over the years tax deferred. Will the company have to send me a 1099 form of some sorts? If they don't, how would I figure the cost basis since monthly premiums went both towards the death benefit AND the contract fund amount?? Thanks!

I believe the company is required to send you a 1099, which should give you the info you need. If they do not, you could get the info from them.
