What is the name of your state? Texas

I work in a doctor's office and I am trying to gather information to set in place protocols for collections and small claims court. I have two questions, first I understand that the Statute of Limitations in Texas is 4 years. When does that time start? Is it from the date of last visit, the date of service in which the money is owing, or the date of last verbal contact with the patient?

My second question is, can we pursue collections on the subscriber of the insurance policy or just the guarantor of the account?
