What is the name of your state? New York

My father recently died. I am listed as co-owner on his bank account, municipal bond investment account, and I am also named joint tenant/owner on a mobile home he owned. Does this property pass to me upon his death, avoiding probate? None of these are listed individually in his will, which was written 35 years ago. I was the main resource/caregiver for dad for almost 20 years and my name has been on the accounts for over 10 years.
