What is the name of your state? California

My father, who is over 80 years old, was issued a subpoena to appear in small claims court as a disinterested party. He has trouble walking. He's almost completely deaf even with a hearing aid. He has glaucoma and cannot see very well even with eyeglasses. Is there a form I can file to appear on his behalf? Thank you for any advice you can provide.
