Background: All people live in California and property is in California.

My grandmother owns a home in southern California. She is reaching her last years and wants to presently transfer title on the house to her daughter (my mom) and son (my uncle). She does not want to leave the transfer to the time of her death, but would rather see the property transferred now.
What is the best method of transferring title from my grandmother to her daughter and son, in ORDER TO MIMINIMIZE ANY PRESENT AND FUTURE TAX LIABILITY FOR ALL PARTIES INVOLVED?
What are the potential tax issues? What are the steps needed to transfer title?
Please help. My grandmother wants an answer soon!
