What is the name of your state? WA
Widowed Mother passed away leaving her 4 adult children as her 401k beneficiaries. Who starts that ball rolling? One of her kids or the executrix of her estate?

One of the adult children should contact the 401k company to inform them that she is deceased. The company will tell him/her the procedure which complies with the laws of your state.

In general, though, this type of asset with a named beneficiary goes directly to the beneficiary without the necessity of probate.

Yes, they will require an original or State certified copy of a death certificate, but it is usually sent in with the rest of the paperwork required.

You might want to see if the company has a website. The procedure for claiming of the asset by beneficiaries may be listed there.
