What is the name of your state (only U.S. law)? CA

What type of documents does the doctor legally have to review to make a medical expert report? In our situation, a police report was left out.

Also, their medical expert stated in his report that there was no evidence of isolation. In our hospice care worker's notes, she notes in her charts that our dad was isolated. His report has inaccurate findings. How does this effect the usefulness/validity of his report? And can we discredit it?

So curious?!?
