What is the name of your state (only U.S. law)? NY

Can a notarized statement be used in place of D.R.L. §§115, 115-b if the notarized statement was presented before a judge? The judge appropriately advised the paternal father (whom was represented by his attorney) but there is no court seal on the notarized statement. Can this be used in place of §§115, 115-b for adoption purposes? It has been 10 years since the creation of the notarized statement. Thank you for your assistance.
