What is the name of your state (only U.S. law)? Alaska
An attorney said he would prepare a trust for us for a flat fee of $4000. That seems extreme given that our case would be simple, not complex nor involved. Does this seem even in the realm of fairness for his fee?

Shop around.

I don't know about Alaska, but for $3000 in Virginia I got:

Trusts for me and my wife,
Wills for me and my wife,
Advanced Medical Directive, Medical POA, and General POA's for each other
Transfer of our house into the trusts,
speciment letters to do other transfers into the trust

A lawyer who does a LOT of trusts will probably expend less time and money preparing yours than a lawyer who has to do so de novo.
