locked gates on county roads

I need to know what form I would use to file a civil case in district court. We have an agricluural lease in colorado from the state land board. There is a county road accessing it. It is very old, 100 plus years. A bigt corp bought land near my lease and put gates and locks across the county road. I have been to cunty court, the judge told me I needed to file the gate thing in District. What form do I use?
