Hello. I am in a situation where I develop/create display ads for a desktop publishing company. Many of the ads I create include pictures from sales brochures from
various manufacturers, the products are wide ranging (i.e. cars, boats, kitchen cabinets, etc.).

My question is: Is there a form out there that I could have my customers sign when they supply art to me that would protect me and my company from legal ramifications, thus transferring the responsibility to the person who is giving my the art. The form basically stating that they assume responsibility for the art origin.

Yes, it is called a license. Go to the legal forms section on this site, and you should be able to find it.
