How old is your son?
How long have you worked for this employer?
How many employees does your employer have within 75 miles of your location?
In the 12 months immediately preceding the start of your leave, had you worked a minimum of 1,250 hours?
How much time, TOTAL, did you miss for this reason within the last 12 months?
What reason were you given for the termination?
Were there others let go at the same time?
