What is the name of your state (only U.S. NY)

My lawyer is preparing a package demand to send the insurance..... Can some one tell what is the package demand? what are the procedue? and what it includes with?

Usually it will have a letter saying what the injuries were and how much you are asking, along with evidence such as medical records, photos, and the accident report if there was one.
