Machinery at my place of work caused death to its operator; it was being used under normal operating conditions. The machinery was later found to be basically flawed; it was constructed specifically for my company. Who should be held responsible for the death - my company, the manufacturer of the machinery or the employee of the manufacturer who directly responsibly for the fault?

Usually an attorney will sue all of the above looking for the deep pocket and/or a combination of deep pockets. After all, the attorney usually gets 1/3 plus expenses.
