What is the name of your state (only U.S. law)? Florida

How to handle (documentation, testimonies, etc) a complaint against an attorney where is it believed that he did not disclose an expunged record?

Should the complaint be anonymous? If the name of the complainant is disclosed in the complaint, would this information be provided to the person whom the complaint is against?

Additionally, said attorney was arrested for indecent exposure and possession of drug paraphernalia. Albeit this is a misdemeanor, should it have been reported to the Florida Bar?

If proper disclosures were not carried out, what are the probable consequences for his actions?

Ethics are defined by the State Bar. A letter of complaint should be directed to their Office of Investigations laying out the specifics. Don't use statements like, "I believe." Lay out the evidence in a clear, concise manner. Being "anonymous" could cast doubt about the veracity of the complaint.
