Hi,

I am from Texas, and have recently created a new cologne I would like to market. I would like to take recorded surveys of consumers trying the product and giving their honest opinion, and then use those recording as testimonials for my product.

This is actually similar to the Pepsi Vs. Coke test videos:
YouTube - ‪Pepsi Taste Test commercial [1983]‬‏

After the random testing, Selected customers will give their opinion on a 1 on 1 video, similar to this: Origins | Video

What type of legal contract should I get them to sign, so that I can use their video testimonial for my product online and protect myself? Is there a legal document/template that I can get online, that gives me permission to use their recorded video?

Please advise

-Singh
