Incorporators file an application with the state in which the business is to be incorporated. You can get an application from your state dept. of finance and administration, or even buy one at an office supply store.
State grants corporate charter containing . . .

Name of corporation.
Purpose of its business.
Names and addresses of incorporators.
Names of corporate officers.
Description of capital stock to be issued.

The corporate charter, which is a contract between your state and the incorporator(s), will then be drafted by the state.
