Form 5498-Trad IRA Rollover

I am in California. I have a form 5498 (IRA contribution Info) that came with a form 1099-DIV. The 5498 is for an Traditional IRA rollover. Only line 2 (Rollover contrib.) and line 5 (Total fair mkt value) have amounts in them.

Since I rolled over the full amount on line 2, do I need to show this anywhere on my Federal return for 2001?

Yes, you must show this on your federal return. Show the total amount of the rollover on form 1040, line 15a, Total IRA distributions; and show 0 on line 15b, Taxable amount.

That way you show the rollover, but there is no tax to pay on it.
