How to "Docket a Judgement' in New Jersey

I just won my small claims case and want to docket the judgement in NJ so a lien can be placed on the defendent's property. He's a builder who still owns homes on my block which may close within the next few months. I never received anything in writing from the county I won the small claim in (Monmouth). Is there a form I need to fill which would then be sent to the Superior Court Clerk's office in Trenton in order to docket the judgement?

Any help would be greatly appreciated.
