I have my judgement - now what

What is the name of your state (NY);

Hi all,
so i took my LL to court after my security deposit was not returned. I have a judgement in my favor, and I issued an information subpoena to the LL's bank, and have confirmation that an account is still maintained there.

Now here's my question:
The information subpoena was sent to the bank's legal department - but as I am filling out the 'Request for an execution" for the Sheriff, it asks for Debtor's Bank and Bank address.
There are two branches of this bank in the area where the LL lives - do i have to do some old fashioned detective work to find out which bank address I should provide? the subpoena was answered in yes/no format, so no bank account/address was provided. How should i handle this - please advice.
