Can anyone help?

I'm filing a lawsuit in lieu of a debt to me and am on the step of the Default Judgment request form. I've only got a clue as to how to fill this out, and I definitely cannot afford a lawyer.

It asks for the money award total including:
1. A money award (Is this my initial requested amount?)
2. Prejudgment interest (What is this?)
3. Costs and service expenses (Would this include the filing fee to the county that is on the small claims notice? Would it also include the $36 fee I paid to the sheriff to serve the papers?
4. Prevailing party fee (What's this?)

The next line:
I request judgment include postjudgment interest rate ___% per__ based on _______[authority for interest]
Is it wise to request interest? What is "authority for interest"?
