I recently received a red light camera ticket in the mail. I was NOT the driver at the time. When the ticket was sent, it included no pictures of the driver. When I went to fill out an affidavit pleading I was not the driver of the vehicle, it immediately had another section listed for me to list the name of the actual driver as well as their address and phone number. (www.violationinfo.com)

My questions:
1) Do I have to put a name down of the person who was driving (there was no picture given) in order to prove my innocence.
2) If I do not, would I try to leave the "required" sections blank or would I need to call to set up a court date?

Thank you for the help!

Kelsey
