What is the name of your state (only U.S. law)? Arizona

Is there such a thing as an AKA form { "also known as" form } that can be used in conjunction with a liability waiver form?

Example: Upon joining a club, members sign a one time Membership Liability Form and on that form they put their legal name as well as "aka so and so" ... Now as a member of said club they attend an event and are required to sign an Event Liability Form, can they use their alias or must it too be signed with a legal name?

They need to sign with their legal names. You can register a fictitious name in many jurisdictions, but you still need to sign under your name, i.e.,

John Smith d.b.a. Spoon Diggity Dog.
