Changing from Sole Proprietor to LLC?

What is the name of your state? Ohio

About 3 years ago, I registered my business as a sole proprietor. Now, things have begun taking shape, and I really want to change it to an LLC. I tried searching online, but the only forms for changing the type of business did not include sole proprietor. Is it possible to get this changed?
 


The reason you can't find any forms is technically (at least in Arizona) sole proprietorship is not a legal entity.

You need to form the LLC with the governmental agency there is in Ohio (some states it's the Secretary of State like CA and NV, in AZ however, it is the Arizona Corporations Commission).
