What is the name of your state? Maryland

Hi All

Do I Need an Operating Agreement with a Single Member LLC , I am ptresently a sole proprietorship and am doing an LLC ( single member ) .
Is an operating agreement required ? , If so where can I find a sample form to view ?

Also in the state of mayland which is where I am filing does the registered agent have to sign on the form or can I just use the Resident agent adress ?
