The 1040 tax form page 54 paragraph 3 for 1999 mentions that I can fill out a tax statement instead of fileing an income tax return. What is an income tax statement?
Ist it another form or a document I write up? If so what must be in it?

There is no IRS fill in the blank form for this. It must be signed in blood.

You can use this form:

INCOME TAX STATEMENT in lieu of form 1040

1. Gross Income for Year 2000 $______
2. Tax income for Year 2000 $______
3. Tax due to IRS (copy #1) $______
