Filing a 5500-EZ for trust

What is the name of your state (only U.S. law)? California

I have a closely held California C corp of which I am the sole employee.
I am filing the year 2008 form 5500-EZ for the Profit Sharing Plan Trust (of which I am the only participant). The trust has less than $100,000 in equity.
Am I required to include an accountants' statement, use an accountant (I can fill out all the information myself-the form is simple), or file any additional paperwork?
In the past I used a CPA who included an additional accounting statement of the PSP Trust with the 5500 EZ.
