I just recieved a W-2

What is the name of your state? Washington, but I am a resident of Wisconsin.

Yesterday, March 7th, my employer handed me a W-2 for 2003 as I was leaving work. I was surprised to be recieving it as I am payed in cash weekly and the only paperwork I ever filled out was a W-4 at the end of December, 10 weeks after I was hired. On this W-4 I claimed 2 exemptions, but my weekly wage was not affected (I am paid minimum wage, less about 7.6% each week). When I informed my employer that legally I should have recieved the W-2 by Jan. 31st, he replied that it has been sitting in his office since then, and nobody on the staff had requested them. I work 4 times a week, and my employer is presently every shift.
I have kept records of my wages, and this W-2 does not reflect the correct income earned. I recieved from him forms: Copy B and C stapled together.
Box 1 shows $327 less then my gross wage for 2003, consequently SS and Medicare tax are less.
Box 2 shows $0 income tax witheld.
The State, Employer state ID#, State wages, and State income tax are all empty (Boxes 15-17).
Also, my SSN is correct, but my last name is incorrect, and there is no address for me.
The major problem is that I have already filed my Federal and WI state taxes using the W-2s I had recieved previous to yesterday.
I am a WI resident, and am returning to the state soon. I know my relationship with this employer has been illegal, but it was the only job I could find after 6 weeks of looking. I am again searching for a new job, but would rather keep this one until I have been hired elsewhere.
Any advice to my options on changing this W-2 and/or ammending my 2003 tax returns will be greatfully appreciated.

If you lived and worked in Washington, there won't be any state info on a W-2 because WA has no state income tax.

That your last name is incorrect doesn't make much difference.

You haven't indicated whether you filed and included the income from this job, or not.

If you tell us exactly what you did, we can answer the rest of your questions more accurately.

Thank you Snipes.
I have already filed, and did not include the income recieved from this job. The money I made illegallly in WA was a fraction of the income I earned legally in WI in 2003. I was being paid in cash and it was apparent to me that the 7.6% he would subtract form my pay each week was not actually going anyplace but to him. In all honesty, when January and February came and went I assumed that I would not be recieving a W2 from this job and filed without the income declared (the federal and WI refunds are just sitting in my checking account now until I figure out what to do).

So, I need to know the next step because I lied on my taxes and this W-2 is wrong (Box 1 reads 1120.00 when I actually made 1447.25, and tax witheld totals 85.00, when he actually witheld 120.00). I am unsure of what to do at this point.
Thank you, Tracy

Ask your employer to give you a corrected W-2 , and then amend your return.
