What is the name of your state (only U.S. law)? Indiana

I took some money out of my Roth IRA in 2009 and failed to report it on my taxes.
I received a notice from the IRS and I have to prove that I was allowed to withdraw the money.
How can I prove that I was entitled to the withdrawal, followed all the rules, and did so legally?

Were you over 59 1/2 at the time the withdrawal was made?
Did you declare the withdrawn money in excess of your contributions as taxable income and pay the tax on it?
