Sole Proprietor unsure whether to submit SSN to Health Insurance Co for PatientWhat is the name of your state? New York My friend is a sole proprieter (schedule C filing) whose alternative health service is in its infancy. He gave receipts to one of his patients, did not include his SSN on them of course. Now the patient's insurnace company is requesting he complete a W9 to provide them with his Tax ID number. Is his SSN something he must provide? |

| No. He should provide them with his EIN. If he doesn't have one of those, he can easily get one online. I believe the W-9 makes provision for either an EIN or an SSN. He should not use his SSN. Snipes
