unable to get w2 from employer

I live in Wisconsin. I am self employed and contract out. I recently had a falling out with the last fellow I contracted with and now He will not provide me with a statement of earnings or w2. My paychecks were from his private checking account so I have no stubs. Is he required to provide me with a statement of earnings or w2?
