W-2 Form from company i never worked at


What is the name of your state? IL

I received a W-2 Form from a company based in Denver Colorado which i never worked at:

I have no idea where it came from and what it is, I do not have any contact information with them except their address.

I tried Googling them without any luck

This is their address on my W-2 Form:

Elbert Inc
2000 S Colo Blvd #2-800
Denver Co 802222

Now they are claiming i had a gross pay of about 5k in 2007, but i have no recollection of working there at all.

all of my other W-2 form Jobs have come in and are accounted for


Where should i start with this investigation?

What do you guys think this is all about?

Anyone have this issue?

Try a reverse address lookup for the address without the suite number. You might find someone in the building who can get you in touch with a property manager who can tell you who might actually be in that suite.

This is the one thing that makes me say "hmmm".
I, for one, KNOW that I never worked out of state. Was this just a "slip of the tongue", so to speak?

Welcome to the world of identity theft.

More than likely they have hired an illegal alien who has used your social security number (probably picked at random) on I-9 and W-4 forms for employment.

Contact the IRS. You may need to fill out a form 4582 to document that this is not your W-2. From the IRS web site:

I received an incorrect W-2 form. I can't get my former employer to issue a corrected W-2? What should I do?

If your attempts to have an incorrect Form W-2 (PDF) corrected by your employer are unsuccessful and it is after February 15th, contact the IRS at (800) 829-1040. An IRS representative can initiate a Form W-2 (PDF) complaint. Form 4598, Form W-2 or 1099 Not Received or Incorrect, will be sent to the employer and a copy will be sent to you along with Form 4852 (PDF),

Identify theft is a possibility. But, chances are he may have worked for a subsidiary of the above places that I found with that address.

I agree, because the W2 came to his address. Usually when identity theft is involved, you don't find out about it until you get a CP-2000 letter from the IRS. Its not impossible that its identity theft, but its a bit more likely that he worked for a subsidiary.
