W9 Question

I am the custodian for the savings account of my minor child, (Florida, if it matters). We're talking only about a $300 balance or so.
I received a W9 in the mail from Bank of America today requesting the form be completed and returned.
I have two questions:
1) Part 1 asks for a social security number. Is that mine or the child's?
2) Will I need to file a tax return for the child next year?

It should be the child's info, and no, the account will not generate enough income for the child to be required to file a tax return.
