What is the name of your state (only U.S. law)? TX

I am trying to fill out my advance directive. There is a section for additional requests. I want it to be perfectly clear that in the event of injury or illness that causes me to need assisted care or any type of life support, if my quality of life is diminished and I can't take care of myself, I do not want any treatment.

My problem is, I don't know how to say that and be perfectly clear.

Can anyone give me suggestions?

Further in addition to the written directives, make sure all those who are close to you know your desires and know where the directives are.
They do you no good if you leave them in the safe deposit box.

I have heard of a person getting a tattoo on their chest to avoid exactly that

Some people will add what hospital(s)/hospice they wish to be treated in (and those they do not want) ... as well as specific Doctors they would want.
Some may have specific directions regarding their known illnesses.
Some have requests regarding using their own blood, or relatives only, etc.
(ps: 'need assisted care' is very broad terminology).
... These are additional 'requests'...

ya mean the part where it states that after consulting with your doctor...?


I think that was a hint they would like you to talk to doc about it. He will be able to assist you in writing what you want in a manner that is understandable by those that are bound by this. You want to avoid confusing those folks. He may be able to bring up circumstances or treatment you have not thought of but should be addressed.
