My husband died in April 2000. My minor son is one of two beneficiaries on a small account at a local credit union.
In order for me to retrieve the money on his behalf, I was
told that I would have to show proof of guardianship. What
form do I need to file? In Probate/Surrogate Court?

Ask the credit union people. If it is small most credit unions will not be an obstacle and somebody there will probably show you exactly what would satisfy them, and tell you what you need and tell you how to get it in your state.
