What is the name of your state? Alabama
I am in the process of preparing a Living Will and Health Care Proxy for myself. Do I have to identify it as an "Alabama" Living Will" or can I write one that is generic and applicable to other states? Although my residence is in the state of Alabama, my designated Agent and Health Care Proxy reside in the state of Maryland. Also, the document will be endorsed by a Notary Public licensed by the state of Maryland. Will this present a problem?

Also, is it appropriate to have someone in the health care field designated as an alternate Agent if he/she is not personally associated with my health care?
