What is the name of your state? Pennsylvania.

Hello,
I am the only custodian of a UTMA. I would like to transfer my rights upon my death to another. Here's the situation:
The minor is my neice and her grandmother who is much younger then me, takes care of here. Most of the money in my neice's account has been deposited from my own funds. The grandmother just gave me $100.00 towards the account.
So I figured that if she trust's me, then I should trust her. Is there any legal way for her to be named successor custodian upon my death without the need for a will or trust?
A limited power of attorney would not work in my State because it's revoked upon my death.
