I was injured on the job, which resulted in a Longshoreman's Workman's comp claim. I was paid for my time off during the recovery of 3 surgeries over the course of 2 years, and have since returned to work in a limited capacity in the office rather than in the field. I went to see the doctor for a routine status update on my ankle, and she said I needed another surgery.

A new salesman with the company had overhead me talking about how the injury hurt again, and he called the insurance company and delivered a false statement about how I re-injured it, and discussed with the insurance company the option to offer me a settlement to resign. I would like to know if any HIPAA laws were broken in this exchange, and if so, which ones?

Now the insurance company does not want to cover my surgery, and wants to send me to a doctor of their choosing, which will undoubably not be in my best interest.

I would like to know if I have a right to review all documents involving my claim, including the cost of each surgery, physical therapy, and compensation for the temporary total disability paid to me during my recovery. I would like to see every document I have signed at the different hospitals. Additionally, I would like to see every statement that any employee had delivered to the insurance company regarding my injury and supposed re-injury

If I do have the right to see all of this information, how is the best way to ask for it or obtain it?
