Auto accidents related to Workman Comp. case

What is the name of your state (only U.S. law)? CALIFORNIA

I have a Workermans Comp. case which involves injury to my back (neck, thoracic & lumbar). I've reported one car accident I can remember within the past 10 years, how far must I recall ANY accidents? I don't want the D attorney bringing up something I can't remember? I've checked with the CA DMV and they only keep records back so far (drop off after 3 years).

My questions are: How far back must I report having an accident? And, where can I find records that list ALL my accidents?What is the name of your state (only U.S. law)?
