What is the name of your state? Arizona

Is an orthopedic Doctor bound by Doctory client privilege like attorneys are?, where what you say to him can't be repeated? To Case workers and etc?? Sorry don't know the correct terminology of that term

Yes, in general, but there are a lot of exceptions, in some cases you might have to sign a release in others not. It would be better to give an example.

well we have another visit with the Dr next thurs, and want to talk about how the company actually is and their mentality, and how the insurance company attitude is about the injury,

If this is workers comp, 3rd parties can have access to at least some of your confidential records, ask the doctor to define ahead of time what is confidential, remember you may agree that something is confidental, but if you have a legal claim, opposing side may get a court order to release the information any way. Also you can record your sessions for your own records.

Great, thank you much, I will ask him. In the past we've allowed the case worker to be present, since we wont to be able to speak free, we're going to ask she wait outside this time. The case worker has always said he understood if we didn't want him in the room, but we always have allowed him.. But you never know if they're all in the same boat together, Case worker and insurance company.. Hope that doesn't look bad to deny him this time. We just want to speak freely, but will find out what's confidential. Thanks

This is in re records etc, it seems a bit unusual for a case worker to come to your appointments with you or are they providing transportation because you can't drive?

No my wife is the one injured, The case worker just comes because it's assigned to him. The insurance company is trying to deny her WC I think ,saying they don't know if it's the same injury and will want a re-evaluation to determine??? WTF,, Yes it's the same injury, it's starting back hurting and she followed rules, reported it and now they seem to want to play dirty, not sure but is smells like it..

That is way too intrusive, get a worker's comp attorney, Monday! Figure they are probably video taping as well or the case worker has something else going on.
